SCHEDULING A NEW APPOINTMENT/EVALUATION
When calling to schedule an appointment, please specify whether you are a self pay patient or whether you have insurance. If you have insurance please be prepared to provide the office staff with all of your insurance information. Our new patient appointments are scheduled for 40 minutes and can be in office or over the phone. Our staff is available to take your calls and schedule an appointment Monday through Friday. If we do not immediately answer your phone call, please leave a message and we will call you back as soon as possible.

FIRST APPOINTMENT CHECKLIST:
-Arrive 15 minutes before your scheduled appointment time to fill out new patient paperwork
-Bring your insurance card and your ID
-Payment in full (Cash and Card are accepted)

REFILL POLICY:
During a patients follow up appointment they are given enough medication to get them through to their next appointment. Therefore, it is imperative that patients schedule all follow-up appointment so as to not run out of medication. It is always the patient's responsibility to call the office in a timely manor when they are running out of medication. All controlled substances will be pending in the system for up to 24 hours NO EXCEPTIONS. Controlled substances are not to be sent in early for any reason. If the patient is out of the window for a follow-up appointment and are out of medication, the patient is required to schedule an follow- up appointment and in order to send in any medication there will be a $15 refill fee collected. When a patient pays for a $15 refill there will be enough medication sent to get the patient to their next scheduled appointment.

NO SHOW POLICY:
We understand that sometimes patients must reschedule appointments or will have a conflict arise. Therefore, out of respect for our other patients and our physicians time, we allow you to reschedule or cancel any appointment up to 24 hours before your appointment time. If the patient does not show up or is unable to be contacted for the appointment it will be marked as a no show. If a patient calls the same day of their scheduled appointment to cancel or reschedule it will be considered last minute. After 2 occurrences within the same year, there will be a $50 fee collected at the next appointment.
